These registration forms can be downloaded and filled out for registering canes or sticks sold by a member of the American National Cane Club. The forms are kept by the ANCC President as a record of the sale/history of the cane or stick. One copy of the form goes to the person buying the cane, one copy gets sent to the ANCC President and another copy is kept by the cane club member.
